Student Concerns and Opportunities Form

The CEHD Student Concerns and Opportunities Form is a key tool in CEHD’s commitment to continuous improvement, by allowing students to submit feedback, concerns, or commendations in a formal, standardized manner.

Steps Before Submitting the Form

Before submitting the form to College Administration, you must attempt to resolve the issue at the lowest level possible. Think of it as a mandatory three-step escalation:

STEP 1

Direct Conversation: For issues with a course, a grade, or an advising session, you must first speak with the instructor, professor, or advisor directly. Most issues are successfully resolved right here.

Step 2

Department Level: If that conversation doesn’t lead to a resolution, you then escalate the matter to the Department representative (typically the associate department head) or the appropriate Program Coordinator.

Step 3

College Administration: If the issue is still unresolved after taking the first two steps, use the formal CEHD Concerns and Opportunities Form below. The form sends your report directly to the Associate Dean’s office for review and action.
